To Enroll in Elase Academy

1. Fill out and submit the online Application for Enrollment by clicking this link: http://elaseacademy.com/student-academy/application/
2. OR Download the Application For Enrollment:

3. Complete the application form and submit with $50 application fee:
   • via Paypal (follow link online)
   • via fax 801-208-5740
   • via e-mail at theacademy@elase.com
   • in person at Elase Academy
4. Contact our Admissions Office to set up a Financial Assistance meeting.
5. Submit all required documentation at least 30 days prior to your desired start date.
6. Once you have been accepted for enrollment, a deposit may be required to hold your place at the Academy.
